Payment and Consent Form

Parent/Guardian & Student must complete this form together.  Have your credit card information ready as you will be prompted to enter payment information at the bottom of the consent form.  Please read each section of the form carefully and fill in all required fields.  You will have the option at the bottom to pay online OR to send in cash or money order.  EVERYONE must fill out this permission form.

There is a section for parents and a section for students, however the email address to fill out this form MUST be the one of a parent, NOT a student.  We have included important information for the trip below.

Trip Information:

Date - Friday, May 9th, 2025

Location: Great Adventure  (1 Six Flags Blvd, Jackson Township, NJ 08527)

Time: Students will arrive at school for homeroom and leave approximately 9:00am.  Students are expected to arrive back at school around 4:30/5:00pm and must arrange for a ride home from there.

Cost: $125

The cost of the trip is $125.00 per student, which includes:

                   - Roundtrip transportation via deluxe modern motorcoach

                                                    - Admission to Six Flags Great Adventure with unlimited rides

                                                   - Meal Voucher: Lunch, 1 snack, 1 drink

             - All fees, taxes & gratuities

              - Bus parking pass

- School Trip T-Shirt

 

YOU MUST HAVE PAID SENIOR DUES IN ORDER FOR YOUR CHILD TO ATTEND THE TRIP.  If you have any issues, concerns, or questions about this, you may contact the individuals listed below.  

We will provide more information regarding arrival and dismissal procedures when we get closer to the date.  

Please Note:

*This is a complete package deal.  Season Passes can not be used.

*This purchase is non-refundable.  In the event you are unable to attend the trip, you will receive your ticket and lunch voucher.  Payment for buses, taxes and gratuity cannot be refunded.

 

Payment Information:

Due Date: Monday, March 17th, 2025. Tickets must be purchased in advance. Late payments will not be accepted.

 

Online Payment Option: Complete the online permission slip and select the credit card payment option.

 

Cash or Money Order Payment Option:  Complete the online permission slip and select the cash/money order option. Payments can be submitted to Mrs. Corio in room 218 during Homeroom. Money Orders should be made payable to I.S. 24 School Fund**PERSONAL CHECKS WILL NOT BE ACCEPTED**

Trip Success Criteria

(Required for Participation)

I understand this event is by invitation only and if I receive one, it may be removed at the discretion of the I.S. 24 school administration.

My parent/guardian and I must have an NYC School Account

If I have conduct grades of N or U in any Marking Period (report cards), my invitation will be re-evaluated by I.S. 24 school administration.  

I will have no Classroom Removals, Principal’s Suspensions and/or Superintendent’s Suspensions.

I agree that in the event of an emergency, injury or illness, the staff member(s) in charge of the trip may act on my behalf and at my expense in obtaining medical treatment for my child.*
